Additionally, on your demo I noticed I have to “login” in order to book appointments. That might be a problem for customers to do that no?

Yes, it’s possible to add contact form shortcode to Home page.
Unfortunately I’m not really sure what do you mean by ‘hero image’, could you please specify that and illustrate with screenshot?
Also you can try to re-built contact form plugin with any 3rd part plugin (for example Contact Form 7).
There is existed layout feature, so you can add login/logout feature, create My Account page and provide all required information to your customer.
